$20.00•25.00 per hour About Us Seva Senior Center is an Adult Day Health Care (ADHC) center dedicated to improving the physical, emotional, and social well-being of seniors through compassionate, person-centered care. Responsibilities Complete psychosocial assessments and participant evaluations Develop and update individualized care plans Provide counseling and emotional support to participants and families Coordinate community resources and referrals Participate in interdisciplinary team meetings Maintain accurate and timely documentation Ensure compliance with
