Summary The Wraparound Facilitator position is full-time and works out of our Yuba City office. This position facilitates a family-centered, strength-based, culturally sensitive, needs-driven planning process for creating individualized services and supports for children, youth, and their families. Ensure the provision of Intensive Care Coordination (ICC) that is responsible for facilitating assessment, care planning, and coordination of service, including urgent services, for children/youth who meet the Katie A. subclass criteria. The Wraparound Facilitator assures access to natural, professional, and community-based options, activities, and opportunities. Essential Duties and Responsibilities Facilitation of and participation in Katie A. Child and Family Team (CFT) Meetings. Providing Intensive Care Coordination (ICC) that is responsible for facilitating assessment, care planning and coordination of service, including urgent services, for children/youth Ensuring the provision of Intensive Home-Based Services (IHBS) Incorporating Strengthening Families Approach and the Five Protective Factors Framework, Trauma-Informed Service Delivery, Nurtured Heart Approach and Safety Organized Practice throughout the Wraparound process Ensuring clients receive access to mental health services, including assessment and individualized treatment that creates a coherent and all-inclusive approach to service planning and delivery Education and/or Experience A Bachelors degree in health or human services or related field from accredited college or university required. At least three (3) years of experience with ED children, adolescents, and families, preferred. Bilingual is a bonus, but not required Possess a valid State of California Driver License in good standing The estimated base pay for the Facilitator is $22.00 to $29.00; additional individual compensation may be available.
