The Social Worker will assume responsibility for a caseload of approximately 25-40 adult participants, most of whom have one or more medical diagnoses. The Social Worker will provide a range of services including case management, community resource referrals, counseling to elderly and other physically/mentally impaired adults in a day program setting.

Requirements:

• Bachelor's degree in Social Work from an accredited school with 2 years of experience providing social services in one or more of the fields of aging, health, or long-term care services OR
• Master's degree in Social Work from an accredited school of social work OR
• Master's degree in Psychology, Gerontology, or Counseling from an accredited school and 1 year of experience providing social services in one or more of the fields of aging, health, or long-term care services
• Valid Driver's License and proof of insurance

Responsibilities:

• Conduct biopsychosocial assessments (includes home assessment)
• Case management (schedule appointments, coordinate transportation, assist with access to community resources, etc.)
• Develop and implement person-centered treatment plans, with the ability to determine appropriate milestones to gauge progress
• Provide individual and family counseling
• Attend multidisciplinary team meetings, communicate updates to relevant parties
• Make referrals to other healthcare providers as needed
• Complete all documentation and reports in a timely and accurate manner
• Community outings

• Intakes and discharges

Qualifications:

• Must be energetic, self-motivated, organized, and efficient with completing tasks and meeting deadlines
• Must be comfortable working in a culturally-diverse setting, and utilizing on-site translation services when necessary.
• Must have excellent oral and written communication skills, with basic proficiency in Microsoft Office (additional computer program training will be provided
• Must be willing to assume additional job duties as assigned; center staff operates as a team, and all staff fill in with additional tasks when necessary (assisting with activities, meal service, answering telephones, filing, etc)
• Knowledge of community resources in Modesto and surrounding areas a plus
• Bilingual preferred, but not required (Spanish)
