We are actively seeking a compassionate, dedicated, and organized Transitional Housing Case Manager to join our team in Redding. In this role, you'll play a vital part in helping youth and families navigate critical life transitions — supporting them toward safety, stability, and success.

What You'll Do:

Carry a caseload of clients and provide personalized, wraparound support Work closely with biological families, foster families, and county social workers Coordinate and attend key meetings (FCT, CFT, TPM, FARE, FPM, etc.) Maintain accurate, timely case notes and documentation Support foster parents and address client-specific needs Supervise and monitor family visits as needed. Complete SAFE Home Studies and assist with home re-certifications Be available for on-call support as needed Follow all confidentiality laws and policies with care and professionalism
