Through our integrated Foster Family Agency and Counseling Center, we offer a continuum of care that centers the voices and experiences of those we serve.
Program Overview:
Counseling Center The Counseling Center at A Coming of Age Foster Family Agency provides comprehensive mental health and supportive services to children, youth, and their families across Riverside and San Bernardino County. Rooted in trauma-informed care and cultural humility, our multidisciplinary team delivers services in homes, schools, community settings, and our central office. This approach ensures flexibility, accessibility, and meaningful engagement within natural environments. Position Summary The CFTM Facilitator is an essential member of the Counseling Center team, responsible for coordinating, preparing for, facilitating, documenting Child and Family Team Meetings, and completing the Integrated Practice Child and Adolescent Needs and Strengths assessment, also known as the IP-CANS, for children, youth, and families involved in the child welfare system. The CFTM Facilitator works collaboratively with county social workers, caregivers, youth, family members, service providers, and natural supports to ensure meetings are organized, inclusive, trauma-informed, and focused on the strengths, needs, safety, permanency, and wellbeing of the child and family. This position requires strong communication, organization, assessment, documentation, and engagement skills. The CFTM Facilitator supports the full meeting process, including referral review, outreach, pre-meeting preparation, information gathering, completion of the IP-CANS assessment, facilitation of the CFTM, development of action steps, and timely submission of required documentation within program and contract timelines. Key Responsibilities Review referrals and assigned cases to identify required timelines, meeting needs, participants, assessment requirements, and documentation expectations. Coordinate with county social workers, caregivers, youth, family members, service providers, and natural supports to schedule Child and Family Team Meetings within required timeframes. Conduct timely outreach to all identified team members, including caregivers, youth, county partners, service providers, and other relevant supports. Complete pre-meeting preparation with families and team members to gather information, clarify concerns, identify strengths, and support meaningful participation in the CFTM process. Complete the IP-CANS assessment by gathering, organizing, and synthesizing information related to child and caregiver strengths, behavioral and emotional needs, life functioning, risk behaviors, cultural considerations, trauma experiences, and caregiver resources and needs. Use information gathered through referral documents, team member input, caregiver and youth engagement, collateral contacts, and the CFTM process to inform accurate IP-CANS ratings and documentation. Facilitate Child and Family Team Meetings in a structured, trauma-informed, culturally responsive, and family-centered manner. Promote the active participation of youth, caregivers, parents, relatives, natural supports, and professional team members during the meeting process. Support the team in identifying strengths, needs, safety concerns, permanency goals, behavioral health needs, family supports, and actionable next steps. Ensure that the IP-CANS assessment, CFTM discussion, and action plan are aligned with the child and family's identified strengths, needs, priorities, and service recommendations. Prepare accurate and timely CFTM summaries, action plans, IP-CANS assessments, service recommendations, and related documentation in accordance with agency and contract expectations. Submit required documentation to the supervisor within established timelines, including post-meeting summaries, action plans, completed IP-CANS assessments, and supporting materials. Maintain consistent communication with supervisors regarding case progress, barriers, scheduling challenges, assessment needs, escalation concerns, and documentation status. Identify and communicate concerns related to safety, engagement, service barriers, placement stability, permanency planning, behavioral health needs, or unmet family needs. Maintain organized records of outreach attempts, coordination contacts, meeting schedules, participant communication, assessment information, and completed documentation. Participate in team meetings, case consultations, training, and supervision as required. Complete required training and certification related to the
IP-CANS, CFTM
facilitation, documentation standards, and contract expectations. Uphold professional boundaries and adhere to agency policies, ethical standards, confidentiality requirements, mandated reporting laws, and safety protocols across all aspects of service delivery. Minimum Qualifications High school diploma or equivalent required. Experience working with children, youth, families, or caregivers involved in child welfare, behavioral health, foster care, education, or other child-serving systems. Familiarity with Child and Family Team Meetings, child welfare processes, family engagement, care coordination, or multidisciplinary team collaboration. Strong interpersonal, facilitation, communication, and conflict-resolution skills. Ability to engage individuals from diverse backgrounds with professionalism, empathy, and cultural humility. Strong organizational skills and ability to manage multiple referrals, deadlines, meetings, and documentation requirements. Ability to work independently and as part of a multidisciplinary team. Ability to maintain confidentiality and professional boundaries. Ability to pass background checks, including criminal record clearance, Child Abuse Index, and FBI fingerprinting. Proficiency with technology, virtual meeting platforms, documentation systems, email communication, and Microsoft Office or Google Workspace. Preferred Qualifications Bachelor's degree in social work, psychology, counseling, human services, sociology, child development, or a related field preferred. Bilingual English/Spanish strongly preferred. Experience facilitating meetings with families, caregivers, youth, county partners, and multidisciplinary teams. Experience completing IP-CANS, CANS, care coordination documentation, behavioral health documentation, or related assessment tools preferred. Work Environment & Physical Demands This position may involve both in-office and community-based work, including meetings in homes, schools, county offices, community settings, virtual platforms, and the agency office. Candidates must be able to travel locally, manage flexible scheduling needs, and navigate a variety of environments. Some meetings may require availability outside of traditional business hours to accommodate family and team schedules. Physical requirements include the ability to lift up to 20 lbs occasionally and perform basic physical tasks such as sitting, standing, climbing stairs, and reaching.
