We currently have the following position open working out of our Riverside office: Facilitator (Case Manager) - Wraparound Program Wraparound is a home based services program designed to help families at risk overcome obstacles and remain united. Facilitators must guide the Child and Family Teams in the development of Plans of Care and Crisis and Safety Plans according to Wraparound timelines. (Full-time, 40 hours weekly) In this key position you will facilitate the Wraparound process by maintaining highly coordinated communication with all family members, system partners, and other individuals involved in the life of the child and family. To be successful in this position, you must be a self-starter who also works well as part of a team. Candidates should be detail oriented with excellent organizational and communication skills, as well as a team player who is able to apply sound reasoning and good judgment in a wide variety of diverse situations. Previous Wraparound experience extremely helpful. Must have previous relevant experience and a BA/BS (MA/MS preferred) in a related field. A CA driver's license with a good driving record, reliable transportation, and the ability to pass a criminal background clearance are also required.
