JOB DESCRIPTION POSITION TITLE

WORKER REPORTS TO
JOB SUMMARY:
Provides services, support and guidance to adult patients with end-stage renal disease, their caregivers, and their families in outpatient environment. Performs diagnostic evaluation, individual, family & group therapeutic counseling, and resource referral. Maintains comprehensive knowledge of the Centers for Medicare and Medicaid Services (CMS), Network 18, and Health Department regulatory requirements. May be required to work some weekend shifts and holidays

EDUCATION, EXPERIENCE, TRAINING
Master's degree in social work. Two years experience in clinical setting, previous counseling experience preferred. English literacy Bilingual in either Mandarin or Spanish Proficient computer skills
A. KNOWLEDGE OF WORK
Physical and psychological effects of End Stage Renal Disease Human Behavior Theories and their application Strong observation, analytical and listening skills Strong communication skills Patient management according to CMS benchmarks Principle of infection prevention and control Confidentiality, compliance, patient/provider rights, corporate and professional codes of conduct, HIPAA privacy standards. Conflict resolutions Team building strategies
B. DUTIES AND RESPONSIBILITIES
Monitor patients psychological status, assesses changes, and intervenes accordingly. Provide mental health counseling to patients and if necessary, their families. Interview and evaluate patients' problems, provides information and counseling support as necessary. Develop treatment plans with patients by engaging patients to take an active part in their own treatment. Administer social service programs. Conduct annual Quality of Life surveys and document in accordance with State and Federal Guidelines Conduct annual Patient Satisfaction surveys and document in accordance with State and Federal Guidelines Assist with concrete needs and refer to resources such as insurance coverage, medication assistance, and mental health care. Assist and collaborate with transplant center Assist patients in completing applications for additional financial coverage, such as Medicaid, and state or special programs; provide follow-up on determinations of eligibility; and document proof of continuing eligibility. Provide additional assistant resources for patients as appropriate. Keep accurate treatment records for each patient Participate in multidisciplinary teams and meetings Participate in the Facility Safety Program Assist in orientation and in-services as appropriate Function as part of the Quality Assurance and Improvement team Assist with reporting requirements and activities put forth by the Renal Network Participate in training and in-services as necessary Act as consultant or resources in the area of expertise for other members in the healthcare team Maintain continuing education including participation in professional organizations related to End Stage Renal Disease (ESRD) patient care management. Perform other duties as assigned or required.
