This is a hybrid clinical + coordination role: you'll carry a direct caseload while taking ownership of day-to-day team operations — caseload distribution, scheduling oversight, compliance checks, and new staff onboarding. It's designed for a social worker ready to step into leadership without stepping out of direct practice. Lead Responsibilities Assign and balance caseloads across the social work team; monitor for coverage gaps Facilitate weekly team huddles and coordinate department communication Review and maintain initial assessment scheduling for regulatory compliance Support onboarding of new social work staff; orient to documentation standards and workflows Escalate performance or conduct concerns to the Program Director/Admin Clinical Responsibilities Provides full-time coverage during all center hours of service Orients visitors, families, and new participants to
ADHC/CBAS
staff, program, and facilities Completes Enrollment Process; collects eligibility data (medical, financial) from participant/caregiver Develops individual care plans with the interdisciplinary team using Person-Centered Planning; implements and documents interventions same-day per the IPC; provides psychoeducation, crisis intervention, and psychiatric referrals as needed (per 42 CFR 441.301(c)(1)-(3)) Conducts pre-enrollment home environment assessments Communicates regularly with families; supports continuity of care at home Provides information, referrals, and liaison services between participants/families and social service agencies Develops and updates individualized discharge plans aligned with MDT assessments Maintains accurate, timely documentation: progress notes, quarterly reports, and memos Reassesses continuing/returning participants; prepares updated treatment plans Develops and facilitates support groups for participants, families, and caregivers Organizes outreach programs for participant recruitment; monitors and finalizes the intake process Contributes to in-service training development and delivery Supports external resource coordination and internal utilization review Department Coordination Coordinates Social Work department activity — caseloads, documentation compliance, scheduling — under the direction of the Program Director/Administrator. Performance review and disciplinary authority are retained by Program Director at this level. Growth Path High-performing candidates may be eligible for promotion to Social Work Supervisor within 6 months, which includes expanded performance management and disciplinary responsibilities. Requirements (All applicants must meet one of the following pathways) MSW, or Master's in Psychology, Gerontology, or Counseling from an accredited school + 1 year of experience in aging, health, or long-term care — OR — Bachelor's in Social Work from an accredited school + 2 years of social services experience — OR — Licensed Clinical Social Worker (LCSW), Board of Behavioral Sciences Other Qualifications Mandarin — Required | Cantonese — Preferred Case management experience with elderly populations Knowledge of physical, mental, and social needs of older adults Multidisciplinary team experience Strong written communication skills Benefits Monthly Bonus | Signing bonus (subject to availability) Dental & Vision insurance | 401(k) | Retirement plan Paid Time Off | Professional development assistance
