Coordinate community resources such as Medicaid, Medicare, Section 8 housing, and disability services to meet individual needs. Support patients with behavioral health challenges and addiction counseling when appropriate, ensuring holistic care. Assist with client communication by explaining treatment options, documenting progress accurately, and maintaining detailed case records. Collaborate with interdisciplinary teams to develop personalized care plans that respect cultural diversity and individual preferences. Offer mentoring and guidance to colleagues on social work best practices and workforce development initiatives. Engage in ongoing training related to first aid, CPR certification, and working with geriatric or disabled populations to enhance service quality. Skills Strong experience in crisis management and crisis intervention strategies within healthcare or social services settings. Proficiency in addiction counseling, behavioral health support, and working with seniors or geriatric patients. Excellent writing skills for documentation, reports, and client communication that are clear and comprehensive. Ability to identify resources effectively within social services systems to support diverse client needs. Bilingual abilities are highly valued for serving culturally diverse communities with sensitivity and respect. Skilled in motivational interviewing techniques to foster client engagement and positive outcomes. Knowledge of Medicaid, Medicare, Section 8 housing programs, and disability services is essential for resource navigation. Experience working with individuals with disabilities enhances the ability to deliver inclusive social work services. Demonstrated capability in case management, client advocacy, mentoring colleagues, and supporting workforce development initiatives.
