Social Services Supervisor | PACE Program Clinical Magnet , a division of ICON Medical Network, is looking for a compassionate, dedicated Social Services Supervisor to join our partner's full-time team in San Bernardino, California . The Social Services Supervisor provides leadership, oversight, and advanced guidance for the delivery of psychosocial services within the interdisciplinary team (IDT). The Supervisor ensures that Master's‑level Social Workers (MSWs) complete all regulatory assessments, care plans, and documentation in accordance with PACE, state, and internal standard requirements, and that delegated tasks completed by Social Worker Assistants (SWAs) meet the same quality and compliance expectations. Sales Representative Responsibilities o Leadership, Supervision, and Clinical Oversight o Regulatory, Process, and Quality Compliance o Participant Care Coordination and Complex Case Consultation o Health, Safety, and Housing Oversight o Communication, Documentation, and Coordination o Administrative Leadership Key Performance Indicators The Social Services Supervisor is accountable for both team and individual performance outcomes, including: Regulatory and documentation compliance (timely assessments, care plans, SDRs, grievances, incident documentation). Performance management of all staff on key indicators benchmarks, as defined in performance addendum. Housing, safety, and SDOH outcomes including risk mitigation and reduction of unresolved high‑risk concerns. IDT integration and participant experience, reflected through positive feedback on collaboration and communication. Team performance, engagement, retention, and completion of performance reviews. The Supervisor's own caseload documentation quality, timeliness, and modeling of trauma‑informed, person‑center practice. Compensation $82,800 - $103,500 REQUIRED Education Master's degree from a school of social work accredited by the Council on Social Work Education Licensure, Certification, Registration or Designation Hold minimum state certification/registration (e.g., LSW) for master's level social worker Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) from The California Board of Social Work-LMSW License Meets all state requirements for practicing social work in healthcare settings (including supervision where applicable) Require personal transportation, current state issued driver's license, good driving record and auto insurance as required by law. CPR/BLS and First Aid as required by state Work Experience and Qualifications Two (2) years' supervising staff Three (3) years' social work experience in medical/healthcare setting conducting assessments, care planning, and case management One (1) year experience with complex geriatric/adult populations with multimorbid medical and psychosocial conditions One (1) year experience identifying and responding to health and safety issues, housing instability, and social determinants of health Two (2) years' experience conducting psychosocial assessments, risk assessment, and developing care plans Knowledge of state, county, and local governmental and community-based resources. Extensive knowledge of Medicare, Medicaid/Medi-Cal, and dually eligible benefits and services Familiarity with medical terminology, common conditions, common medications, experience with older adults with complex multimorbid medical issues
