Compensation $82,800 - $103,500 REQUIRED Education Master's degree from a school of social work accredited by the Council on Social Work Education Licensure, Certification, Registration or Designation Hold minimum state certification/registration (e.g., LSW) for master's level social worker Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) from The California Board of Social Work-LMSW License Meets all state requirements for practicing social work in healthcare settings (including supervision where applicable) Require personal transportation, current state issued driver's license, good driving record and auto insurance as required by law. CPR/BLS and First Aid as required by state Work Experience and Qualifications Two (2) years' supervising staff Three (3) years' social work experience in medical/healthcare setting conducting assessments, care planning, and case management One (1) year experience with complex geriatric/adult populations with multimorbid medical and psychosocial conditions One (1) year experience identifying and responding to health and safety issues, housing instability, and social determinants of health Two (2) years' experience conducting psychosocial assessments, risk assessment, and developing care plans Knowledge of state, county, and local governmental and community-based resources.
