Job Description
ASAP:
Social Worker We are seeking a Social Worker responsible for the assessments and care planning of clients with multiple medical and psychosocial problems. Social Worker plans for and monitors services, interventions ensuring provision of quality care, plays role of a liaison between participant, family, caregivers, professionals, and participates in community outreach efforts.Educational requirements:
Master's degree in Social Work, Psychology, Gerontology, or Counseling from an accredited school is required. Experience providing social services in one or more of the fields of aging, health, or long-term care services. Or Bachelor's degree in Social Work from an accredited school is required Experience providing social services in one or more of the fields of aging, health, or long-term care services. Skills & Experience 1. Good communication and presentation skills. 2. Demonstrates case management skills and experience in the community health care delivery system. 3. Detail oriented with good problem-solving skills and the ability to prioritize multiple tasks. 4. Experience with community outreach. 5. Computer literacy very important. 6.CBAS/ADHC
