POSITION SUMMARY
One Community Action (OCA) is seeking a highly dedicated, motivated, team-oriented, and experienced Case Manager Specialist to join our
POR VIDA!
program. In partnership with the Santa Maria Joint Union High School District, the
POR VIDA!
program provides wraparound services, eliminates barriers to access, and coordinates available resources for improvement in the following: Socio-emotional well-being, cultural pride, and sense of belonging. Graduation rates, attendance, and grades. Goal-setting & college-readiness. Accessing therapy, tutoring, and other services. Under the direction and supervision of the Youth Services Manager, the case managers are committed to OCA's mission as we work to meet the needs of students, parents/caregivers, and school sites.
KEY RESPONSIBILITIES
Serve as primary on-site staff at assigned school locations, providing high school students with a safe and healthy environment. Provide support and a safe space to at-promise students, including system-impacted, houseless, English learners, and foster youth. Collaborate with school administrators and staff from assigned schools to effectively serve students referred to the program in mentoring, engagement, and goal setting. Provide individual case management services in English and Spanish to referred students. Conduct parent/student assessments as required. Facilitate and co-facilitate groups and workshops using a culturally responsive curriculum, and deliver pre- and post-surveys. Collect program impact data using qualitative (testimonios) and quantitative (surveys) methods. Support students in improving grades and attendance, being on track to graduation, and achieving short-term goals. Maintain strong documentation of services provided and student progress, supporting program evaluation activities with surveys and reports using Aeries and Casebook. Conduct phone calls, texts, and emails to communicate with students/parents. Make referrals for students' educational needs and other adjunct services. Maintain regular contact with assigned school administrators and provide support with monthly reports. Observe scheduled work hours. Be punctual and prepared. Maintain strong communication with parents, coordinate parent group and individual meetings. Ensure the confidentiality of all parents, students, and staff. Attend staff meetings, training as required, and events that may take place on a weekend. Attend school staff meetings, especially with academic counselors, and participate in giving updates or gaining clarity about your caseload. Outreach to students, parents, and the community through tabling or presentations. Recognize, document, and take appropriate action for suspected child abuse and neglect, as a California Mandated Reporter (training and guidance provided). Provide transportation to youth as needed. Perform other job functions and duties as assigned.
REQUIRED SKILLS & QUALIFICATIONS
Must have an Associate Degree in Human Services or a related field. Some education can be substituted for on a year-by-year basis. Bachelor's Degree preferred. Bicultural and Bilingual in Spanish & English, required . Speaking an Indigenous language is a plus. Minimum (2) years of experience working with youth or related fields in a non-profit setting. Behave ethically-have a strong understanding of ethical behavior and business practices, and ensure that your behavior and the behavior of others are consistent with these principles and align with the values of the organization. Must have a valid California Driver's License and Liability Auto Insurance. Must pass a Department of Justice background check and TB test. Experience with Microsoft Office (Word, Excel, PowerPoint). Experience with Zoom, Google Suite, and other digital technologies.
PREFERRED SKILLS & QUALIFICATIONS
Knowledge of racial equity and trauma-informed approaches. Experience working with system-impacted, houseless, foster, at-promise youth. Have a strong understanding of the root cause of systemic oppression, and or be open to learning more about such values, and embrace the agency's stance on such issues. Demonstrated skills and success working with BIPOC (Black, Indigenous, People of Color) communities and the ability to create a safe space for students of BIPOC communities. Drug & Alcohol Counselor Certification. CPR and First Aid Certification preferred, or the ability to complete training. Experience with National Compadres Network curriculum preferred: Joven Noble, Cara y Corazón, and Girasol. Experience with group facilitation with topics related to violence, healthy relationships, culture, and identity. Knowledge and understanding of data-entry practices

Resourceful, well-organized, and able to solve practical problems and manage various situations. Ability to respond quickly and appropriately to an emergency or a crisis. High energy levels, ability to work independently with limited supervision. Strong commitment to doing what is right for students. Displays sound judgment in all situations, especially when working with students and families.
PHYSICAL CONDITIONS AND WORK ENVIRONMENT
Requires daily standing, sitting, bending, and stooping. Requires the ability to lift 50 pounds. Adaptable and able to work weekends & evenings as needed. Ability to travel independently as needed for work-related matters. A reliable mode of transportation is required.
