MOZAIC HOSPICE
Hospice Social Worker Position Summary The Hospice Social Worker is a key member of the interdisciplinary hospice team, providing psychosocial assessment, counseling, emotional support, resource coordination, advocacy, and education to patients and families facing serious illness and end-of-life care needs. The Hospice Social Worker collaborates closely with physicians, nurses, chaplains, hospice aides, volunteers, and community partners to ensure comprehensive, compassionate care that honors the wishes, dignity, and values of each patient and family. Essential Duties and Responsibilities
  • Conduct comprehensive psychosocial assessments of patients and families within required regulatory timeframes.
  • Identify emotional, social, financial, cultural, spiritual, caregiver, and environmental factors impacting care.
  • Develop, implement, and update psychosocial plans of care.
  • Provide supportive counseling, crisis intervention, and caregiver support.
  • Facilitate goals of care and advance care planning discussions.
  • Connect patients and families with community resources and support services.
  • Participate actively in interdisciplinary team meetings and care planning.
  • Provide bereavement support and identify families at risk for complicated grief.
  • Facilitate support groups for patients, caregivers, and bereaved family members as appropriate.
  • Participate in community outreach and education activities.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Adhere to NASW Code of Ethics and all regulatory requirements. Education
  • Master's Degree in Social Work from a CSWE-accredited program. Experience
  • Hospice and palliative care experience preferred. Licensure and Certifications
  • LMSW or LCSW in the State of Connecticut.
  • Valid driver's license and reliable transportation.
  • Maintains all required licensure and continuing education requirements. Physical and Mental Requirements
  • Ability to sit, stand, walk, bend, and travel throughout the service area.
  • Ability to communicate effectively in person, by telephone, and electronically.
  • Ability to manage multiple priorities while maintaining attention to detail.
  • Ability to respond appropriately in emotionally challenging situations.
  • Ability to complete required documentation and computer-based work. Environmental Conditions
  • Travel to patient homes, assisted living communities, nursing facilities, and healthcare settings.
  • Possible exposure to infectious diseases, blood, body fluids, and environmental hazards.
  • Exposure to emotionally sensitive situations involving serious illness, death, grief, and family crisis.
