CCH Social Worker Community Residences - 2.9 Newington, CT Job Details Full-time $21.27 an hour 1 hour ago Benefits Flexible schedule Qualifications Collaborate with healthcare professionals Working with individuals with developmental disabilities Caseload management Writing skills Mid-level Organizational skills Documentation review Home visits (communication methods) Full Job Description As a CCH Social Work Case Manager SW/CM, you are responsible for supporting individuals with developmental disabilities who are living in DDS licensed private homes located throughout the State of Connecticut. In this role, the SW/CM is required to visit each home on their assigned caseload a minimum of one time per month. During each monthly home visit, the SW/CM will spend time with the individual(s) living in the home and their care provider. The SW/CM will be responsible to review the individuals personal file which contain medical, financial, and behavioral information and complete documentation following each visit within required timeframes. This role will work closely with a support team that includes a Nurse, Behaviorist, Supervisor, and DDS Case Manager to ensure that the individuals in your homes have continued access to the happy and healthy lifestyle that they deserve. In this role, the SW/CM will have the opportunity for a flexible schedule (Monday through Friday) in conjunction with the needs of the program. Office hours will be worked in our Main Office location of Newington. The ideal candidate for this role is someone who is a self-motivated 'people person,' is flexible, and possesses strong organizational skills. Being a team player, as well as having strong writing skills are also preferred skills. Mon-Fri Flex 40
