This is a unique opportunity to join a mission-driven healthcare organization and play a key role in a program focused on helping seniors age safely and independently in their homes. We are looking for a professional who brings both clinical expertise and a passion for advocacy , and who enjoys guiding families through complex care decisions related to aging, chronic illness, and dementia. This role offers autonomy, meaningful client relationships, and the opportunity to make a direct impact in the lives of older adults and their caregivers. The Role As a Geriatric Care Manager, you will work directly with older adults and their families to assess needs, coordinate services, and develop personalized care plans that support safety, independence, and quality of life. You will serve as a trusted advisor and advocate , helping families navigate healthcare systems, community resources, and long-term care decisions. Key Responsibilities Conduct comprehensive psychosocial and care assessments in clients' homes Develop individualized care plans addressing medical, emotional, and social needs Coordinate healthcare services and community supports Provide guidance and counseling to families navigating caregiving challenges Advocate for client preferences and quality of life Support families dealing with dementia, chronic illness, and aging transitions Collaborate with physicians, home health clinicians, and community partners Participate in interdisciplinary care conferences and program development Who This Role Is Ideal For This position is well suited for professionals who: Enjoy building long-term relationships with clients and families Are passionate about aging services and dementia care Prefer field-based, relationship-centered work rather than institutional settings Value flexibility and autonomy in their work Want to help shape a program within a respected healthcare organization Qualifications Candidates must have one of the following: Master of Social Work (MSW) Licensed Clinical Social Worker (LCSW) - Connecticut Certified Geriatric Care Manager (CGCM) Registered Nurse (RN) or Licensed Practical Nurse (LPN) with CGCM certification Minimum 2 years of experience working with older adults Experience with dementia care, caregiving support, or chronic illness management Background in care coordination, case management, or community-based services Additional Requirements Certification through NACCM (or ability to obtain within six months of hire) Valid driver's license and ability to travel throughout Fairfield County, CT Strong communication, advocacy, and relationship-building skills Why Join RVNAhealth? Compensation:
The pay range for this position is $75,000 to $78,000 per year and represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
