The nonprofit's work is guided by its belief that "living well" involves accessing resources that can improve health and offering opportunities for continued purpose, social connection, growth and belonging. Are you passionate about leadership in long-term care? Do you find joy in serving those living with cognitive changes due to dementia? Calling all social workers who are ready to take the next step in their career! We are seeking a special kind of social worker to join our transdisciplinary leadership team to support 60 residents, their families and staff. This transdisciplinary team will consist of leaders who possess a compliment of skills and experience in nursing, social work, art and recreation, and occupational therapy. The Social Work Co-Leader will be a coach and mentor of staff and build their capacity to enhance elder well-being through a holistic, strengths-based approach to services and supports. The Social Work Co-leader will foster an environment and culture that enables residents to maximize their individuality, independence and dignity. The ideal candidate holds empathy and compassion for all and is someone who believes it is possible to live a life of meaning, purpose and joy at all stages of dementia. Requisite skills and duties for this position includes: Experience leading teams Ability to model approach to build capacity in others A high degree of empathy for others Emotional intelligence Enjoys a collaborative work enviroment Ability to be self-aware and reflective Self-directed Attention to details and strong organizational skills Strong communication and listening skills Work with residents to identify/assess psychosocial, emotional and spiritual needs and provide, develop, and/or aid in the access of services and supports to meet those needs Provide support and counseling to residents and families experiencing and/or anticipating issues to adjusting to illness, changes in living situations and bereavement Play a key leadership role on the transdisciplinary team in the development and implementation of a "LiveWell" life plan for each resident that meets the psychosocial, emotional, physical and spiritual goals of each resident and their end of life wishes. Foster a trauma informed care approach to providing care and services to residents and families Educate residents and families on their rights and responsibilities and educate staff on the rights of residents and their role in supporting each resident's well-being. Ability to flex hours to support staff - salaried position generally M-F and occasional weekend day.
REQUIREMENTS
Master's degree in Social Work preferred Minimum of 2 years post master's experience in a medical setting State of
CT, LCSW
preferred. Experience in long-term care desired. Experience working with electronic medical records, MDS and care plans preferred.
