Through advanced practice skills mobilizes resources to reduce risk and functions as ambassador between Hospital and community. The Social Worker is responsible for providing high quality patient care, displaying good skills in assessment, treatment planning, individual and family intervention, and discharge planning. The Social Worker functions well as a member of the multidisciplinary team.

Qualifications Minimum:

Bachelors or Masters Prepared Social Worker; Bachelors or Masters prepared Rehab Counselor, or Bachelors or Masters Prepared Mental Health Counselor Experience One (1) year experience in social work services which includes care planning and referrals to community agencies, community/public setting or medical social work necessary Certifications LCSW is only required for student supervision American Heart Association BLS Organization Description Delray Medical Center, part of the Palm Beach Health Network, is a 536-bed acute care hospital spanning 30 acres, employing over 1,700 employees, 700 physicians and serving southern Palm Beach County for over 40 years.
