Job Overview We are seeking a compassionate and dedicated Healthy Start Peer Outreach Worker to connect with pregnant and postpartum women, fathers of infants/toddlers, and/or expectant fathers facing behavioral health challenges, including mental health conditions and substance use disorders. This role involves engaging with diverse populations, providing support, and facilitating access to services. The ideal candidate will have experience in crisis intervention, crisis management, and behavioral health, with a strong commitment to promoting recovery and well-being. Responsibilities Conduct outreach activities to engage pregnant and postpartum women, fathers and expectant fathers experiencing mental health or substance use issues in various community settings. Provide crisis intervention and crisis management support to individuals in distress, ensuring safety and stabilization. Offer behavioral health counseling, utilizing motivational interviewing techniques to encourage positive change. Assist clients with navigating mental health, addiction services, and social support systems, including harm reduction strategies. Maintain accurate documentation of interactions and service delivery using Electronic Health Record (EHR) systems. Collaborate with healthcare providers, social workers, and community organizations to coordinate care plans. Mentor clients through recovery processes, fostering resilience and self-efficacy. Work with individuals who have co-occurring disorders, tailoring interventions to meet complex needs. Educate clients about available resources and advocate on their behalf for access to treatment and support services. Skills Proven experience in crisis intervention and crisis management within behavioral health or social services settings. Strong understanding of behavioral health principles, mental health counseling, and addiction counseling techniques. Experience working with individuals with co-occurring disorders or mental health conditions. Excellent mentoring skills with the ability to build rapport and trust with diverse populations. Proficiency in using EHR systems for documentation and case management. Knowledge of harm reduction approaches and motivational interviewing strategies. Ability to handle sensitive situations with empathy, professionalism, and cultural competence. Effective communication skills—both verbal and written—and the ability to collaborate within multidisciplinary teams.
