Summary:

The Social Worker is a key member of the interdisciplinary team who provides and oversees the provision of psychosocial care for selected patients and families. Conducts bio-psychosocial assessments, provides intervention and treatment as indicated. Identifies high‑risk psychosocial factors of patients/families that impact health status and helps staff understand the influence of those factors upon the course of medical care. Collaborates with and provides social work consultation to other disciplines within the healthcare setting and the community. Preferred experience in behavioral health.

Responsibilities:

Maintains patient lists and patient data tracking for outcome reporting.

Develops and maintains daily activity schedules.

Provides community outreach and engagement as needed.

Provides case management and clinical services for clients and their families.

Evaluates and assesses the needs and strengths of clients and families.

Facilitates groups with adults and/or adolescents.

Must demonstrate the ability to provide social work services to patient, family members and significant others in four areas: 1) psychological functioning, 2) adaptation to illness, injury, disability or life event, 3) psychosocial assessment/evaluation and 4) facilitation services such as referral community agencies financial planning, and provision of information and resource knowledge. Required to identify and intervene for suspected abuse including sensitivity to age specific concerns: physical/emotional/sexual abuse of children and adolescents; domestic abuse for adults; caretaker/institutional abuse for senior adults. Must be able to manage crisis situations, demonstrating ability to quickly evaluate crisis situation and select appropriate intervention. Must be able to document direct service in patient's medical record after each significant encounter. Must demonstrate proficiency in use of the electronic medical record. Must be able to demonstrate good problem-solving skills, root-cause analysis, and principles of clinical performance improvement.

Required Work Experience:

One (1) year of social work experience in a health care setting.
