A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

Job Description
You will:
Complete comprehensive psychosocial assessments for hospice patients and families. Provide emotional support, counseling, and crisis intervention to patients and caregivers. Assist patients and families with advance care planning, community resources, financial assistance, and long-term care planning . ️ Advocate for patient wishes and help patients and families navigate difficult decisions with compassion and respect. Collaborate with physicians, nurses, chaplains, hospice aides, volunteers, and other members of the interdisciplinary care team. Participate in interdisciplinary team meetings and contribute to the development of individualized plans of care . Educate patients and caregivers about available resources, programs, and benefits. Provide care wherever patients call home, including private residences, assisted living facilities, nursing homes, hospitals, and inpatient hospice settings . Participate in the on-call rotation as assigned. Schedule Monday - Friday 8:00 AM - 5:00 PM Participation in an on-call rotation is required. Holiday rotation is also required. Qualifications ✅ Required Master's Degree in Social Work (MSW) . Valid Florida Driver's License . ️ Reliable transportation and current automobile insurance. Ability and willingness to travel throughout Jackson, Holmes, Washington, Calhoun, Gulf, and Bay Counties as needed to serve patients and families. Preferred Florida Licensed Clinical Social Worker (LCSW) or license-eligible. Two years of experience in hospice, palliative care, home health, oncology, or another healthcare setting . Experience with grief counseling and end-of-life care . Knowledge of community resources, government assistance programs, and other services available to patients and families. Strong communication, counseling, interpersonal, and organizational skills.
