Full-time General Summary:
Responsible for planning, developing, organizing, implementing, evaluating, and directing social service programs of the skilled nursing facility.Position Responsibilities:
Responsible for providing services to meet the social and/or emotional needs that affect our residents' ability to achieve their highest level of function. Participate in the development of the resident's individualized comprehensive plan of care. Participate in QAPI and interdisciplinary meetings. Maintains regular social service progress notes including initial and quarterly assessments. Responsible for all aspects of discharge planning including coordinating discharge meetings, scheduling transportation, coordination of DME, and completing appropriate discharge paperwork. Serves as the Grievance Officer and Abuse Coordinator for the facility. Educates staff regarding resident psychosocial issues. Responsible for accurate and timely completion of PASRR Responsible for completion of Medicaid applications as needed. Provides crisis intervention, emotional support and counseling as needed to residents, families and staff. Assists with residents and family members with Advanced Directives that accurately reflect their wishes. Perform other duties that may be assigned by the Administrator.EDUCATION REQUIREMENTS
Bachelor's Degree or Master's Degree in social workREQUIRED EXPERIENCE
At least two years recent experience in a health care setting Geriatric experience preferred Must possess excellent communication and organization skills Relates positively to residents, families and staff members Basic computer knowledge and ability to learn computer softwareJob Type:
