Social Worker Job Description:

Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living Provide information to the resident, and/or family member, on residents' rights and responsibilities

Qualifications:

Must be bilingual in Spanish (and English) Must have BSW or MSW Must have prior nursing home / long-term care experience Previous experience as a Social Work Director Strong knowledge of discharge planning, care coordination, and resident advocacy Experience working with interdisciplinary teams Familiarity with state and federal regulations related to long-term care #7552
