As a Hospice Social Worker, you will play a critical role in delivering compassionate care to our patients and their families. You will provide emotional, psychological, and social support to patients as they navigate end-of-life care. This position involves working closely with a multidisciplinary team to ensure each patient's needs are met.Key Responsibilities :
Conduct psychosocial assessments of patients and families. Coordinate care and provide counseling, psychotherapy, and support for patients and their loved ones. Intervene in crisis situations to ensure patients receive immediate and appropriate care. Educate patients and families about their treatment plans, hospice services, and available resources/support systems.Service Area :
