Description:
- This Social Worker II position will cover the MercyOne Genesis Hospice House as well as community hospice which will require some driving. The PRN position is 8 hour day shifts Monday - Friday from 8:00am to 4:30pm.
ESSENTIAL FUNCTION
- To provide varied and ongoing social work services to Hospice patients and their families.
This may include psychosocial evaluation, ongoing psychosocial counseling, direct casework services, bereavement services, community outreach and referral through collaboration with physicians, staff, families, community agencies and hospice leadership. Actively participate in quality monitoring and performance improvement activities.

Licensed in Iowa or Illinois required upon hire. Second state license required within 12 months. Valid Driver's License, Auto Insurance-Genesis minimum limits required.Experience:
