CEO SUMMARY/OBJECTIVE

Provides professional team-based care to patients and assists in the continuum of care and assists in educating patients on their patient centered medical home.

ESSENTIAL JOB DUTIES

(This list may not include all of the duties assigned.) Provide mental health services such as therapy, counseling and intervention services to patients. Provides good interpersonal skill as well as the passion and inner drive to help others. Assists with billing out for services performed and completing appropriate documentation in the patient's medical record. Provide assistance, access to different resources and guidance. Attends required meetings and participates in committees as requested. Participates in professional development activities. Performs related work as required.

EDUCATION AND EXPERIENCE

Required:

Master's Degree in Social Work Two years of Post-degree clinical experience
