Job Description

Discover how WesleyLife is redefining what it means to age well: The WesleyLife Way As a Social Worker with WesleyLife At Home, you will: Explain WesleyLife Hospice and Palliative services to patients and families. Visit patient homes (wherever they call home) to assess the psychosocial status, safety concerns, and bereavement risk factors of patients and families/caregivers. Communicate findings to patient care teams to update care plans. Carry out social evaluations and plan interventions. Counsel patients and families/caregivers regarding stress and other identified coping difficulties. Provide information and referral services for patients and families/caregivers regarding financial, practical, and environmental needs. Serve as liaison with patients and families/caregivers and care providers or community agencies. Participate in caregiver, placement, and discharge planning when needed, including discharge to a higher level of care. LISW preferred, LMSW highly encourage to apply.
