Overview We are seeking a compassionate and energetic Licensed Master Social Worker (LMSW) to join our dynamic team. In this vital role, you will deliver high-quality social work services, including clinical assessments, counseling, and care coordination, to support clients with diverse mental health needs. You will be visiting short/long-term rehabilitation centers and assisted living facilities to provide services. Your expertise will help empower individuals to achieve stability and improved quality of life through personalized intervention and community resource linkage. Duties Conduct comprehensive client assessments to identify mental health needs and develop tailored treatment plans. Document all clinical interactions accurately in electronic medical records (EMR systems), adhering to medical documentation standards and HIPAA privacy regulations. Engage in crisis intervention and manage urgent situations with professionalism and sensitivity. Support clients with end-of-life care, hospice care, and aging-related issues, including working with geriatric patients and those with dementia or Alzheimer's disease. Valid licensure as a Master Social Worker (LMSW) or equivalent in the relevant state. Proven experience working with diverse populations, including children, geriatrics, individuals with developmental disabilities or autism, and those receiving hospice or post-acute care. Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, Medicare regulations, and healthcare compliance standards such as HIPAA. Proficiency in clinical counseling techniques including psychotherapy, cognitive behavioral therapy (CBT), motivational interviewing, and behavioral health interventions. Ability to collaborate effectively within interprofessional teams while maintaining high ethical standards aligned with clinical practice guidelines. Excellent client communication skills coupled with cultural competence when working with individuals from diverse backgrounds. Basic life support (BLS) certification including CPR is preferred; additional certifications related to hospice & palliative medicine or assisted living are a plus.
