Know Your Team:
Portneuf Medical Center is a nationally recognized healthcare leader with 205 beds, serving as the region's tertiary care and Level II Trauma Center, housing the Portneuf Heart and Vascular Institute, Portneuf Cancer Center, Portneuf Medical Group, and Portneuf Air Rescue. Responsibilities Assure clinical appropriateness of services and efficient use of resources through collaboration with the entire health care team for acute care patients. Performs medical necessity reviews and initial assessment within 24 hrs of patient admission. Identifies and communicates changes in the patients plan of care to the health care team. Maintains detailed knowledge of community resources and process for referals Completes all assigned UR tasks on a daily basis Using LOS, anticipates patient needs, stewardship of resources and timely transitioning of patients to alternative level of care Documents in patient record in a consistent and timely manner to include initial screenings and discharge plan Screen and refer to appropriate community programs for specific populations
Qualifications Job Requirements:
Current Idaho Social Work license - LSW, MSW, or LCSW Graduate of accredited bacculaureate degree in social work. Minimum of five years of experience, preferably in a hospital setting
Preferred Job Requirements:
Case management certification preferred
