Job Description

Purpose of Position:
The Individual Services Coordinator supports the delivery of individual student services-including disability, at-risk, and mental health services within Heartland Head Start. This role ensures that developmentally appropriate, legally compliant early childhood practices are planned, implemented, and monitored in collaboration with program staff and community partners.
Essential Functions:
Coordination & Compliance
  • Coordinate activities, programs, goals, and policies related to Head Start Performance Standards for disability, at-risk, and mental health services.
  • Review all paperwork, reports, and documentation to ensure accuracy, completeness, and timely submission.
  • Develop and maintain relationships with community agencies in McLean and Livingston Counties to support appropriate services and smooth transitions for children and families. Assessment & Intervention
  • Assist with child assessment processes, including initial screenings and ongoing developmental assessments, to determine the need for referrals for developmental delay or disability evaluations.
  • Provide classroom strategies and behavioral support for identified students.
  • Participate in developmental review activities and weekly team meetings to help staff identify strengths and needs of individual children. Collaboration with Consultants & Districts
  • Coordinate with staff and mental health consultants to develop, implement, and monitor behavior and intervention plans.
  • Meet weekly with mental health consultants to review classroom observations, track records, and update required forms.
  • Collaborate with local school districts and Child & Family Connections (LIC) to support the implementation of Individualized Education Programs (IEPs) and Individualized Family Services Plans (IFSPs).
  • Ensure Heartland Head Start staff attend eligibility, IEP, and IFSP meetings to coordinate services and monitor student progress. Consult with parents and staff regarding the progress and needs of individual children. Training & Support
  • Provide training to staff on disability, at-risk, and mental health services.
  • Support staff in implementing strategies that promote positive behavior and inclusive classroom practices.
Qualifications and Job Requirements:
  • Bachelor's degree in special education, early childhood education, psychology, or a related field, including 21 credit hours in ECE or Child Development and at least 3 semester hours focused on the exceptional child
  • Minimum of two years' experience coordinating or individualizing services for children with disabilities.
  • Strong verbal and written communication skills with the ability to communicate precisely, objectively, and positively.
  • Demonstrated skills in problem solving, conflict resolution, mediation, negotiation, and leadership.
  • Ability to maintain confidentiality, exercise discretion, and demonstrate tact.
  • Proficiency in computer and software applications.
  • Valid Illinois driver's license, personal auto insurance, and reliable transportation.
Work Environment and Physical Requirements:
This position operates in both classroom and office environments, including periods of high activity. The role requires the ability to:
  • Respond quickly to situations involving children
  • Lift children and materials up to 50 lbs
  • Walk, stand, climb stairs, sit on the floor, crouch, stoop, bend, and squat
  • Sit for extended periods
  • Use hands and fingers without restriction
  • Operate tools, objects, and equipment
  • Maintain adequate vision, including close vision and the ability to focus
