Assesses, plans, implements, and evaluates the social, emotional, and psychological needs of sub-acute patients. Coordinates discharge planning through review with patients and families. Residents Rights Knows and respects patient rights. Ensures all protected health information is kept confidential. Reports all complaints made by patients to supervisor. Reports all allegations of resident abuse, neglect and/or misappropriation of patient property. Safety and Sanitation Follows established safety policies and procedures. Observes safety needs of patients as indicated in care plan. Wears and/or uses safety equipment and supplies when indicated and properly trained to use. Demonstrates job-specific knowledge of fire and disaster preparedness during drills or actual situations. Staff Development Attends and participates in scheduled in-service training, educational classes and meetings, including safety committee. Participates in QAA process. Social Service Director Responsibilities Assess, support, and discharge plan all patients of the facility. Identifies and assists with mood, behavior, and psychosocial well-being issues for patients. Oversees Care Plan Meeting process. Maintains repeated contact with patients and their support systems to provide comprehensive updates on patient progress toward goal achievement. Provides ongoing communication with family/patient regarding discharge needs. Utilizes problem-solving skills to assist patient and family in identifying potential barriers to discharge. Provides education, resources and training necessary prior to discharge from the facility. Educates families and patients on the general criteria for Medicare, Medicaid, Hospice benefits, and insurance coverage. Identifies potential financial barriers and collaborates with family and members of the interdisciplinary team to make recommendations for treatment and discharge planning with consideration to financial status, diagnosis, and discharge needs. Participates with physicians, nursing staff and other members of the Interdisciplinary Team in the planning of patient care services, the patients' plan of care and discharge plan. Communicates daily with nursing and all departments regarding admissions, discharges, and transfers. Coordinates the referral of residents and families to appropriate community, social service, home health service, other extended care and rehabilitation facilities and organizations. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Responds to patients, residents, physicians in a prompt, friendly and sympathetic manner. Collects data, performs patient assessments and completes all state required documentation; Enusres compliance with state requirements. Coordinates home care needs by the time of discharge. Documents pertinent conversations with patients' and their families. Assists in effectively managing all required records for the Social Services Department. Demonstrates good organizational skills. Assists with delivery of resident care. Maintains good working relationship within the department and with other departments to enhance patient satisfaction. Performs all aspects of daily work in a manner which contributes to and ensures an environment of resident safety. Prepares and maintains required reports, records, studies and surveys. Attends in-service training and education sessions, as assigned. Performs specific work duties and responsibilities as assigned by supervisor. Coordinates services with other departments and involves other departments. Oversees maintenance and care of any necessary equipment. Other duties as assigned. Education Bachelor's degree in Social Services or related field. Experience 3-5 years of healthcare experience. Knowledge of Federal and State regulations for skilled nursing facilities. Physical Demands Lifting up to 30 pounds maximum with frequent lifting and/or carrying objects weighing up to 20 pounds; Standing, walking, talking, hearing, stooping, kneeling, crouching, reaching, handling, feeling, pushing, fingering, and exerting force to move objects. Travel Up to 25% Working Conditions Both inside and outside, exposure to noise and vibration, heat and temperature, some exposure to disagreeable conditions including odor and infection. Principal Contacts Works closely with patients, family members, volunteers, facility staff, and community representatives.
