Position:

Provide goal-directed in-home counseling/casework interventions to persons sixty years of age or older to improve quality of life and prevent nursing home placements. Community Outreach Case Manager and Senior In-Home Counselor and Outreach Worker:

Complete initial assessments and develop treatment plans based on client's personal goals and presenting needs. Provide counseling services in clients' homes in accordance with assessment information and treatment plan with overall purpose of addressing mental health needs and preventing unnecessary nursing home placements. Conduct in-home visits and inform persons of benefits and services available to them. Assist older persons in gaining access to needed services such as making referrals for them. Document all services in accordance with agency, state, and federal requirements. Conduct collateral contacts with individuals significant to assessed problem situation. Execute referrals to those community resources providing services which address relevant identified problem circumstances, with client's permission.
