Job Summary Greenleaf Consulting Group LLC is seeking a dedicated, compassionate, and motivated Case Manager to join our growing team serving children and families throughout Allen County. This is a field-based position providing structured, goal-oriented, and time-limited services in clients' homes, schools, community settings, courtrooms, and the office as needed. Case Managers work directly with families to strengthen family functioning, promote child safety, and help clients achieve individualized goals. As a Case Manager, you will provide education, advocacy, transportation, crisis intervention, and community resource coordination while collaborating with the Indiana Department of Child Services (DCS) and other community partners to support positive outcomes for children and families. Duties Provide home-based and community-based case management services throughout Allen County. Complete client intakes, family assessments, safety plans, treatment plans, progress reviews, and aftercare planning. Develop and implement individualized service plans to strengthen family functioning and promote self-sufficiency. Provide parenting education, life skills coaching, crisis intervention, conflict resolution, and family stabilization services. Transport clients to appointments, court hearings, visitations, and community resources as needed. Attend court hearings, Child and Family Team Meetings, school meetings, and other required appointments. Collaborate with DCS Family Case Managers, therapists, schools, placement providers, attorneys, medical providers, and community agencies. Connect families with community resources and assist with overcoming barriers to achieving goals. Monitor client progress and adjust interventions as needed. Maintain accurate, timely, and professional documentation in accordance with agency policies and DCS service standards. Manage an assigned caseload while meeting productivity and documentation expectations. Participate in supervision, staff meetings, trainings, and ongoing professional development. Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Criminal Justice, Counseling, Family Studies, or a related human services field preferred. Candidates with a bachelor's degree in an unrelated field may be considered with relevant experience working with children, families, or vulnerable populations. Experience in child welfare, home-based case management, family preservation, behavioral health, juvenile services, or crisis intervention preferred. Knowledge of trauma-informed care, family preservation, child development, and community resources. Ability to learn and implement the Indiana Department of Child Services Practice Model. Strong communication, organizational, documentation, and problem-solving skills. Ability to work independently while managing a community-based caseload. Proficiency with Microsoft Office and electronic documentation systems. Requirements Valid driver's license with a safe driving record. Reliable personal vehicle and current automobile insurance. Ability and willingness to transport clients. Ability to travel throughout Allen County and surrounding counties. Flexibility to work occasional evenings and weekends based on client needs.
