Position Summary :
As a Social Worker at Providence Living Center, your primary responsibility will be to serve as an advocate for our residents and their families. You will work closely with residents and their loved ones to ensure their physical, emotional, and social needs are met. You will be responsible for documenting residents charts, conducting assessments, and coordinating various aspects of care. Your role is crucial in promoting a positive and supportive environment for our residents
Key Responsibilities:
Advocacy and Support Assessment and Documentation Discharge Planning Collaboration Qualifications:
Must be a Licensed Social Worker. Prefer experience in long term care and memory care. Must be able to work with a multidisciplinary team and have excellent written and verbal communication skills. HMO, Medicare and Insurance experience preferred. Must have knowledge of community resources and ensure compliance with all HIPAA regulations.
WORK SCHEDULE
This Full Time health care position works 8-hour shifts . 8am-5pm, with reasonable flexibility.
