Job Description:

The Oncology Social Worker provides assessment, counseling, education and information, resource referrals, and the coordination of services which meet the psychosocial and mental health related needs of adult oncology patients. The Oncology Social Worker should be knowledgeable in the following areas: medical factors and their psychological correlates, psychosocial aspects of cancer, common psychiatric disorders and their management, ethical and cultural aspects of cancer care, and research methods in oncology. The Oncology Social Worker works with a multidisciplinary team including physicians, residents, medical students, nursing staff, pastoral care staff, and staff from other departments/services involved in patient care. The Oncology Social Worker will also work with providers and community agency representatives to develop and implement patient-centered multidisciplinary care plans.

Essential Functions:

Coordinates timely, comprehensive patient psychosocial assessment and contributes to multidisciplinary care plans for adult oncology patients. Reassess plan regularly and revises as needed to ensure continuing appropriateness of plan as patients' conditions and needs change. Documents appropriately and communicates accurately to team members, patients, and families. Identifies risk issues accurately and implements procedures which appropriately address risk concerns (APS, CPS, MIW, duty to warn, etc.). Coordinates care plan and community resources with patients, families, treatment teams, insurers, and providers to maximize best possible outcomes. Implements care plan in compliance with legal and ethical standards, departmental and hospital policies and procedures, utilizing adequate knowledge of financial reimbursement criteria, provider and resource options, and interpretation of medical data. Works closely with internal and external customers. Participates actively in multidisciplinary weekly conferences, committees, assisting with projects and treatment outcome data. Provides crisis intervention, supportive individual and group counseling, and patient advocacy to enhance patient and family ability to cope with the psychosocial and socioeconomic impacts of a cancer diagnosis. Compiles data (including research related data) and submits timely reports on services performed and the outcomes of such services. Evaluates departmental products, procedures, and services systemically to ensure continuing effectiveness and identify needed improvements. Provides education to co-workers, hospital and medical staff, and the community providing information about social work functions, social services-related concepts and services, and community resources. Pursues continuing education by participation in departmental or hospital-wide in-service programs or in classes, workshops, and/ or conferences. Supervises graduate

Additional Job Description:

Education:

Master's degree in social work (MSW or MSSW) from an accredited program required

Experience:

Oncology or healthcare social work experience preferred. Strong clinical skills, including the ability to provide individual and group counseling to patients and families Excellent communication, collaboration, and problem-solving skills Experience in program development, implementation, and evaluation Knowledge of evidence-based practices in oncology social work Ability to work independently and as part of a team

Licensure:

State of Kentucky CSW required KY LCSW preferred, Licensure in the state of Indiana preferred
