Job Description:
The Social Worker (MSW) is a non-clinical social work position responsible for discharge planning, case management, and psychosocial support of patients and their families within the hospital setting. This involves completing psychosocial assessments and interventions to assist patients in coping with their hospitalization, illness, diagnosis, and/or life situation and provide support and education to families on how to support the patient after hospitalization. The Social Worker also provides advice or guidance for any other non-medical problem or barriers to care patients may have that may hinder their rate of recovery or delay discharge and serves as a resource for continuity of care options after discharge. Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation. Provide psychosocial, emotional support, and crisis intervention for patients and/or family members to include but not limited to understanding the patient's diagnosis and determining goals of care; evaluating motivation for change and assisting with movement along the change continuum; alleviating anxieties and concerns regarding illness and changes in condition; promoting self-management of medical and psychosocial care; enhancing or restoring the capacity for social functioning. Collaborate with healthcare providers, including physicians, nurses, therapists, and case managers, to develop and implement integrated care plans that address patients' social and emotional needs. Advocate for patients' rights and preferences within the healthcare system, ensuring access to appropriate services, resources, and support networks. Coordinate referrals to community agencies, social services, and other resources to facilitate continuity of care and address patients' social determinants of health. Recognize indicators of abuse, neglect and exploitation of adults, children and elders, ensures appropriate reporting to authorities, and works in cooperation with protective service agencies to address medical and emotional needs of victims. Maintain working knowledge of and liaison with community agencies; provide information about and linkages to appropriate community services; participate in area activities related to health and social services as appropriate. Discharge planning and transition support. Maintain accurate and timely documentation of social work assessments, interventions, and case management activities in the electronic health record system. Maintain a working knowledge of relevant medical/legal issues that affect patient care, e.g., advance directives, child and elder abuse, guardianship. Serve as a patient advocate. Keep current with social work and health care developments and seek to increase further enhancement of job-related knowledge.
Education Requirements:
