Job Description

Golden Age Healthcare & Rehabilitation is seeking to hire a Social Services Director at it's skilled nursing facility, located in Denham Springs, LA. This position will work on-site and work together with the facilities current social services director to provide quality care to the residents. As the Social Services Director you will evaluate the social needs of each resident admitted and formulate a written plan of care, review and revise plan as necessary. The Social Services Director will build relationships with residents and family members/caretakers, and report any social changes to the representative of the resident. Job Duties Responsible for documenting and tracking resident discharges from the facility to any other long-term care facility, assisted living/retirement community. Assist the Admissions Coordinator in completing admissions agreements. Refer residents to the proper community resource, such as behavioral health. Scheduling and confirming vision, dental, and other services for residents. Build relationships between residents and staff and teach staff to understand and support residents' individual needs. Assist residents with financial and legal matters, such as applying for pensions, referrals to lawyers, and referrals to funeral homes for preplanning arrangements. Job Requirements Bachelor's degree in either: Social work, Sociology or Psychology is preferred. Experience in long term care setting or/and behavioral experience preferred. Strong communication and organizational skills.
