Licensed Masters Social Worker LMSW

Purpose:

The licensed master social worker will assist in the facilitation of care across the continuum for selected patient care needs with a focus on the following: evaluation of psychosocial needs, crisis intervention, development and implementation of appropriate, timely plans of care to include discharge planning; ability to collaborate with case manager, physicians, staff, family and community based resources to enhance and encourage optimal patient outcomes; evaluation of plan of care with appropriate changes, based on plan outcomes.

Qualifications:

Master's degree in Social Work Up-to-date and unencumbered license with the Louisiana State Board of Social Work Examiners Competent in the following: Effective communication Problem-solving Stress management Computer skills Attention to detail Demonstrates flexibility Ability to express ideas both verbally and in writing
