Monthly, Quarterly, and 6 month contacts are required. Ensuring the Eligibility Determination is completed on an annual basis according to regulations. Collecting required information necessary to plan and complete an IFSP. Facilitating the annual IFSP Team Meeting and completing the Annual IFSP. Facilitating and monitoring the provision of services, including revisions and 6 month review Facilitating the Transition events required to support transition from Part C. Compiling and generating reports. Adhering to all Shoulders of Strength, Inc Policies and Procedures Maintain up to 50 client caseload. Qualifications Applicant must have a Bachelor's degree in a human services field. Must have own transportation and insurance coverage. Travel is required.
