Full Job Description Position Summary:
The Wraparound Facilitator is a full-time non-exempt position reporting to the Wraparound Supervisor. The Facilitator delivers a high-fidelity, family-driven wraparound process for youth and families with complex needs. This role coordinates individualized care across multiple child-serving systems, supports families in identifying strengths and goals, and ensures that planning reflects the family's culture, values, and long-term vision. The Facilitator plays a central role in helping youth remain safely in their homes and communities. Essential DutiesResponsibilities Facilitate a high-fidelity wraparound process that provides individualized, coordinated, family-driven care for youth involved in multiple systems and at risk of out-of-home placement. Connect withFamilies and Team Members:
Reach out to new families within 3 days and meet with them face-to-face within 7 days to start building strong relationships. Coordinate the Child and Family Team (CFT) process, ensuring youth, caregivers, natural supports, and providers collaborate to build a strengths-based, culturally responsive Plan of Care. Work with the Independent Evaluator (Licensed Mental Health Professional) to ensure the comprehensive assessment is completed and integrated into the CFT process. Develop an individualized Plan of Care in partnership with the family, aligned with National Wraparound Initiative standards and federal treatment planning requirements (42 CFR 438.208(c)(3)). Maintain a 1:10 caseload of youth/families. Assist youth and families in accessing services and supports, including mental health, social services, educational resources, and community-based supports needed to achieve their goals. Support crisis planning and stabilization, ensuring families have clear, actionable strategies to manage safety concerns. Respond to crises in coordination with the Clinical Supervisor. Maintain accurate, timely documentation including contact notes, Plans of Care, crisis plans, and team meeting summaries.Safeguard Privacy:
Handle all information with the utmost confidentiality, adhering to HIPAA, FERPA, and other privacy standards. Promote family voice and youth voice throughout all phases of the wraparound process.Collaborates with:
Independent Evaluator, Family Support Partners, Youth Support Partners, schools, mental health providers, and natural supports. Other duties as assigned. Minimum Qualifications Bachelor's degree in social, human services, behavioral sciences, or related field; or Bachelor degree in any field with at least two years of full-time relevant experience working with youth, families, or community service programs or LDH-approved equivalent experience may substitute for degree; Master's degree preferred. Valid driver's license, reliable transportation, and ability to travel throughout the region. Ability to work a flexible schedule, including some evenings/weekends, and participate in a rotating on-call schedule as needed. Proficient computer skills in MS Office (MS Word, Excel, and Outlook) Strong communication, team facilitation, and professional writing skills. WAA Compliance Requirements Pass all required criminal background checks. Be eligible for Medicaid/Medicare participation (no sanctions or exclusions) Have no findings on the Louisiana State Adverse Action List Complete TB testing and drug screening Complete AHA-recognized First Aid, CPR, and seizure training (medical professionals exempt) Maintain all required Wraparound training and credentialing.Work Hours:
40 hours per week, flexible schedule includes some evenings and weekends as needed to meet deadlines; regular local travel required.
