Case Manager (RN, LPN or SW- Skilled Nursing / Post-Acute Care) Ledgewood Rehabilitation & Skilled Nursing Center | Massachusetts Ledgewood Rehabilitation & Skilled Nursing Center is seeking a Case Manager to join our team. What You'll Do:

Coordinate ongoing assessments of patient clinical status and level of care Evaluate and adjust care plans to ensure realistic, outcome-driven goals Collaborate with patients, families, interdisciplinary teams, and payors Manage resource utilization including services, consults, and length of stay Coordinates all aspects of discharge planning process and transitions of care Track and analyze outcomes data to improve care delivery Serve as a resource for staff on managed care and regulatory guidelines Maintain accurate, timely, and compliant documentation

What We're Looking For:
