60 The Pediatric Social Worker EX is responsible for providing social work services to patients and families. Performs in accordance with the facility's policies and procedures. Follows the facility's standards for ethical business conduct. Conducts self as a positive role model and team member. Participates in facility committees, meetings, in-services, and activities.
Responsibilities include:
Assesses patient and family psychosocial needs and develops plan of care in concert with physicians, nurses and therapists. Links patient/family to appropriate community resources including information/referral to sources of financial assistance, transportation, food, housing, and support groups. Provides short-term counseling to identify problems and facilitate needed referrals. Serves as patient advocate and liaison with physicians, families and community to ensure continuity of care. Documents patient/family status and history, diagnosis, treatment plan, evaluation results, observations and progress in medical record. Serves as a resource to other staff on community resources and in area of specialty.
Requirements for this role include:
  • Master's degree in social work. Additional course work in area of specialty (e.g., psychiatric, pediatric) as required.
  • Two years' experience in social work with broad background with diverse patient populations, preferably in medical setting. Additional one year experience in area of specialty.
  • Eligible for licensure if required by state or position.
Pay:
$50,523.20
