- and community-based early intervention services to infants and toddlers who have, or are at risk for, developmental delays, as well as their families, in both home and community settings.
The Licensed Social Worker partners closely with parents and caregivers to ensure that services are individualized, meaningful for the family, and grounded in evidence-based practices. Through this collaborative approach, the family\'s priorities, needs, culture, and values are at the forefront of service delivery. The Licensed Social Worker supports the social-emotional development of young children while helping families navigate challenges that may impact their child\'s development and overall well-being. This role focuses on strengthening family capacity, promoting healthy parent-child relationships, connecting families to community resources, and supporting participation in everyday routines and activities.
Responsibilities Include:
Participate in multidisciplinary team assessments of the child\'s developmental status.
- Provide culturally responsive services appropriate for children ages birth to three years and their families.
- Participate in the development of an Individualized Family Service Plan (IFSP) with responsibility for identifying objectives, outcomes, and strategies related to the child\'s development and family priorities.
- Complete progress notes, reports, assessments, and correspondence as required by PDC and DPH.
- Provide individual and group intervention for infants and toddlers from birth to three years old who have developmental delays or who are at risk for delays, with an emphasis on social-emotional development, attachment, self-regulation, communication, play, and family relationships.
- Assess family and child strengths/resources, concerns, and progress.
- Act as Service Coordinator for assigned families by scheduling, developing, implementing, monitoring, and evaluating the intervention plan for each family on caseload, ensuring that appropriate procedures and required timelines are followed.
- Provide supportive professional assistance to families experiencing stressors related to parenting, developmental concerns, medical needs, housing instability, food insecurity, transportation barriers, behavioral challenges, and other factors that may impact child and family well-being.
- Assist families in identifying and accessing community resources, benefits, and support services.
- Promote healthy parent-child interactions and attachment through coaching and relationship-based intervention strategies.
- Support caregivers in understanding child development, behavior, emotional regulation, and developmental expectations.
- Facilitate parent-child interactions to encourage child development and assist families in acting as advocates for their child.
- Collaborate with multidisciplinary team members and community providers to ensure coordinated and effective service delivery.
- Effectively manage required EICS data entry in home and community settings.
- Keep all records timely and updated in EICS as required by PDC and DPH requirements.
