POSITION OVERVIEW A
leading healthcare facility in Stoughton, MA is seeking a compassionate, experienced, and dedicated Social Worker to join its team. The Social Worker will play an important role in supporting residents and their families by addressing psychosocial, emotional, and social needs while helping ensure a high quality of life and person-centered care. The ideal candidate will have experience in skilled nursing, long-term care, or a healthcare setting and a strong commitment to resident advocacy and compassionate care.
KEY RESPONSIBILITIES
Provide comprehensive social work services to residents and their families. Assess residents' psychosocial, emotional, and social needs and develop appropriate interventions. Advocate for residents' rights, dignity, independence, and quality of life. Participate in interdisciplinary care planning and resident care conferences. Assist residents and families with community resources, referrals, and support services. Provide emotional support and appropriate psychosocial interventions. Develop positive relationships with residents, families, staff, healthcare providers, and community resources. Collaborate with nursing, rehabilitation, activities, administration, and other departments to provide coordinated care. Assist residents and families with adjustment to changes in health, living situations, and care needs. Maintain accurate, timely, and confidential documentation in accordance with regulatory and facility requirements. Support behavioral health and psychosocial needs through appropriate resources and referrals. Participate in quality improvement initiatives and facility programs. Help maintain a respectful, compassionate, and supportive environment for residents and families.
QUALIFICATIONS
Bachelor's degree in Social Work, Social Services, or a related field required. Current and valid Massachusetts Social Worker license required. Previous experience in a skilled nursing facility, nursing home, long-term care, or healthcare setting required. Knowledge of psychosocial care, social services, behavioral health, and long-term care regulations. Strong communication, interpersonal, organizational, and problem-solving skills. Excellent resident advocacy and family support skills. Compassionate, professional, and resident-focused approach. Ability to work effectively as part of an interdisciplinary healthcare team. Strong documentation and record-keeping skills. Must be able to pass required background and employment checks.
