Social Services Designee:
Previous Experience as a Social Services Designee preferred Bachelor's degree in social work required Complete psychosocial assessments for residents. Develop and implement individualized care plans. Assist with admissions, transfers, and discharge planning. Provide emotional support and crisis intervention to residents and families. Coordinate community resources and referrals. Advocate for resident rights and quality of life. Participate in interdisciplinary care plan meetings. Maintain accurate documentation in accordance with state and federal regulations. Assist residents with advance directives and healthcare decision-making. Ensure compliance with CMS, state, and facility policies INDOP
