Job Description
Constellation Health is looking for a Compassionate Hospice Bereavement Coordinator To cover Maine On a Part Time Basis (approx. 24 hrs. per week) Constellation Health is looking for a compassionate Bereavement Coordinator to work with hospice patients and their families in their homes. The Bereavement Coordinator is responsible to plan, implement, and maintain a bereavement program to meet the needs of families/caregivers from Constellation Hospice for up to one (1) year following the death of the individual hospice patient.
