Role Description The Licensed Master Social Worker (LMSW) is a full-time, on-site role based in Sterling Heights, MI. This role involves conducting psychosocial assessments, developing and updating care plans, and providing counseling and support to patients and families throughout the rehabilitation process. The LMSW will coordinate discharge planning, connect patients and families with community resources, and collaborate closely with an interdisciplinary team including nursing, therapy, and medical staff. Daily responsibilities also include documentation in accordance with regulatory and facility standards, participation in care conferences, advocacy for patient rights, and support in navigating complex social, emotional, and financial issues. The LMSW is expected to contribute to a positive, inclusive care environment and to uphold professional and ethical standards in all interactions. Qualifications Active Licensed Master Social Worker (LMSW) credential in the state of Michigan and a Master's degree in Social Work from an accredited program. A minimum of two years experience in healthcare, rehabilitation, long-term care, or related settings, with knowledge of discharge planning and community resource coordination. Strong assessment, counseling, and crisis intervention skills, with the ability to support individuals and families facing complex medical and psychosocial needs. Excellent communication, collaboration, and interpersonal skills, including the ability to work effectively within an interdisciplinary care team. Proficiency in documentation, case management, and time management, with attention to regulatory, ethical, and confidentiality standards (e.g., HIPAA). Demonstrated commitment to person-centered care, cultural competency, and non-discriminatory, inclusive practice. Basic computer skills and familiarity with electronic health records; flexibility to adapt to changing patient needs and organizational priorities.
