FIND / FAMILY INTAKE COORDINATOR QUALIFICATIONS

1. A Master's with certification in at least one area of disability. 2. Ability to collaborate with others in the field of Special Education. 3. Ability to work well with staff, parents, and young children. 4. Ability to communicate to local districts in the Child Find process. 5. Ability to present a positive public relation position on behalf of children with special needs. 6. Experience in working with young children as well as those with developmental delays and multiple disabilities. 7. Demonstrated completion of MDE/Office of Early On® Essentials of Early On® Modules. 8. Must pass criminal background check as required by the School Safety Legislation.

RESPONSIBILITIES WILL INCLUDE, BUT NOT BE LIMITED TO

1. Conduct comprehensive Early On evaluations for children ages 0-3, including intake, Multidisciplinary Evaluation Team (MET) assessments, and development of Individualized Family Service Plans (IFSP), as directed by the Administrator. 2. Monitor and manage the Early On referral system, ensuring timely and appropriate responses to concerns from county residents regarding developmental delays or disabilities in young children. 3. Coordinate with parents and guardians to schedule evaluations in a timely manner following receipt of referrals and complete required follow-up documents following Early On guidelines. 4. Collect and maintain Early On referral data for use in grant applications. 5. Process and facilitate the transfer of Early On children from counties outside of Macomb County, ensuring continuity and compliance with program requirements. 6. Collaborate with local school districts and community agencies to support Child Find efforts and ensure identification of eligible children. 7. Implement ongoing outreach (e.g., screenings, campaigns, referral systems) to inform the public and professionals about available services and how to refer children. 8. Perform additional duties as assigned by the Administrator.
