Responsibilities Community Resource Assessment :

This involves researching existing resources, conducting interviews with community members and stakeholders, and visiting service providers to evaluate the quality of services offered.
Client Needs Assessment :
Social workers assess the needs of individual clients or families by conducting comprehensive assessments, including identifying strengths, challenges, and goals. They determine which community resources are most appropriate and beneficial for addressing the clients' needs and goals.
Referral and Coordination :
Based on their assessments, social workers make referrals to appropriate community resources and services that align with clients' needs and preferences. They facilitate connections between clients and service providers, ensuring smooth transitions and continuity of care. Case Management :
