  • $2069.00 Social Worker needed asap SW with 2 years experience in Case Management and Discharge Planning in an Acute Care setting
  • The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management and financial management, while also completing medical record documentation.
