Where You'll Work CHI St. Francis Health is a ministry serving the head of the Red River basin, located in Breckenridge Minnesota, founded by the Franciscan Sisters of Little Falls in 1899. Job Summary and Responsibilities As our Social Worker you will proactively assess, develop, and implement comprehensive plans of care in collaboration with patients, families, and an interdisciplinary team. Essential Functions Completes psychosocial assessments, interviews patients, relatives, friends or significant others to obtain information about the patient's personal, social and emotional history. Provides therapeutic care to patients to assist them in accomplishing treatment goals using interventions and techniques that are appropriate to the age and developmental level of the patient. Coordinates treatment with community agencies such as social services, schools and probation. Maintains social service records of each patient according to hospital and departmental program standards. Maintains statistics and turns in reports in a timely manner. Communicates with crisis team for mental health emergencies. Coordinates outpatient treatment following inpatient hospitalization Assists in revision of policies and provide input to the departmental budget by identifying departmental supply and equipment needs to meet departmental goals.
